Our minimum age requirement is 12 years old, and patients must have all permanent teeth present. For patients under the age of 18, we need a parent/guardian signature on the Medical/Dental History Form provided during the evaluation step.
Yes. If your dental plan provides orthodontic benefits you may be reimbursed directly from your insurance company after purchasing your aligners. SmileDirectClub doesn't accept direct payment from insurance companies. We can get you the forms you need to file your claim for reimbursement.
